Each week during school closure, there will be a meal request form available for anyone 18 and under to receive breakfast and lunch. Each person receiving a meal MUST fill out the form so we can have an accurate count on the meals to prepare.
Good afternoon: I have spoke with Angela and also called Nancy and left a voicemail on how to handle the day care lunches. Do I need to fill out a form for each child and if so how do you want me to note that it is for the daycare? Also, a required field is where to pick up, which location would you like me to use?
